Hand washing was performed before dressing change. The old dressing was removed. Mild serosanguineous drainage was noted on the dressing removed. The skin on bilateral feet appeared very dry and scaly. Peripheral pulses were poorly appreciated in BLE. Very poor sensitivity in both feet, owing to the poorly managed diabetes. The wound site is on the undersurface of right foot. The surrounding tissue is mildly erythematous and tender. The entire foot and the wound site on the right foot were cleansed with sterile NS and 4 x 4. The wound site was blot dry gently using sterile 4 x 4.  Wound measurements were taken using a sterile Q tip. A piece of hydrofera blue was cut from the sheet and was thoroughly moistened with sterile saline. Excess saline was squeezed out from the piece of hydrofera blue. Cover the site using a sterile 4 x 4, and secured in place using Medipore tape. The right foot was wrapped in kerlix roll gauze. Patient tolerated the procedure well with pain reported at 4 on a scale of 0 – 10.
