Patient/caregiver was educated on contraindications for using Cyclobenzaprine as follows:

  1. Cyclobenzaprine use can result in difficulty with the flow of fluid in the eye that helps to maintain the eye pressure. This blockage in the flow of fluid in eye can contribute to increased pressure in the eye. Poor control of eye pressure can lead to retinal damage and blurred vision. Individuals with history of glaucoma (elevated eye pressure) must be aware of this side-effect of cyclobenzaprine use. Cyclobenzaprine could best be avoided for a safer alternative or used with extra caution in such individuals by observing compliance with periodical vision exam for early detection of any retinal damage and blurring of vision.
  2. Cyclobenzaprine can induce difficulty with emptying the bladder and consequent retention of urine in the bladder, which can contribute to increased risk for urinary tract infections (UTIs). Individuals with history of prostate enlargement and spinal injury can present with difficulty emptying bladder and urinary retention. Cyclobenzaprine use in such individuals can add up to the risk for urinary retention and so, could be avoided for a safer alternative or used cautiously with close watch for symptoms of urinary retention and increased frequency of urinary tract infections.
  3. Serotonin is one chemical in the brain, which in permissible amounts, is believed to reduce anxiety and promote happiness and well-being. Very excessive concentrations of serotonin in the brain can lead to serotonin syndrome. Individuals with serotonin syndrome can present with mania, excitement, hallucinations, agitation, insomnia, pounding heartbeat, very rapid irregular heart rate or arrhythmia, seizures, overactive reflexes, and diarrhea. Cyclobenzaprine use can contribute to increase in the serotonin levels and lead to development of serotonin syndrome. Cyclobenzaprine use, in individuals with history of serotonin syndrome could be avoided for a safer alternative or used cautiously with close watch for symptoms of the same. Also, cyclobenzaprine use, especially along with a few other antidepressants, such as, citalopram, fluoxetine, and sertraline, can increase the risk for serotonin syndrome many fold and so, must be avoided.
