Patient/caregiver was educated on precautions to be exercised while taking Topamax as follows:

  1. Topamax use can result in poor motor coordination and unsteadiness, with increased fall risk and accidents. Confusion and dizziness with Topamax use, can add to the motor incoordination, thus enhancing the fall risk significantly. Observing compliance with general fall prevention measures and DME recommended, such as, cane or walker, can help prevent falls and injuries.
  2. Topamax use can result in reduced sweating and compromised means of heat dissipation from the body, thus leading to extremely elevated body temperature with consequent dehydration and in severe cases might as well present with a heat stroke. Avoid heavy physical exercise, while taking Topamax, as physical exercise can elevate the body temperature and contribute to the possibility of dehydration and heat stroke caused by Topamax. Individuals taking Topamax must also avoid hot environments, such as, outings on warm summer days, sauna rooms and hot tubs, to prevent development of hyperthermia and dehydration. Dressing loosely in light-colored clothing, observing compliance with fluid resuscitation measures recommended and maintaining cold environmental temperatures, while taking Topamax on a regular basis, can help prevent development of hyperthermia and dehydration responses due to the medication. Watch closely for signs and symptoms of dehydration and elevated body temperature. Report any episodes of unresolved elevated body temperature, dry mouth, rapid heart rate, diminished urine volume with highly concentrated dark yellow color to the urine, and dry skin.
